Answer
First you will need to create SYSTEM DSN ODBC entry for your MS SQL database. Then select in SQL2GSM as connection protocol "ado" with port "0". IP, password and username use the same like in ODBC entry. For database name set the ODBC entry name (the real database name can be different).
